What is Empty Nose Syndrome or ENS?

Empty nose syndrome (ENS) is an iatrogenic nasal disorder that occurs when too much of the nasal turbinates are resected. The turbinates have many important roles in the respiratory system. They offer protection of heat exchange, humidification and filtration to the nose the pharynx, larynx, trachea and lungs, and help to preserve the body from dehydration, by cooling down and trapping most of the water vapor that is retrieved from the lungs upon exhalation, and there is even evidence that they help to keep the brain from over heating.

Dear Fellow ENS Sufferer

I felt prompted to share this letter with you, to display Dr. Friedman's compassion, dedication and concern for patients with ENS. I've helped assist with the study, and have visited the clinic frequently in the past year. My name is Paul and I'm the administrator of : enshope@gmail.com

There are many people who know me, but for the ones that don't, I have worked, supported, encouraged and brought hope to many, that contend with this condition. I have also helped establish free medical air charters with Angel Flight and Lifeline Pilots for ENS patients. This enables each individual to participate in the Mayo Clinic's study and should not limit anyone from getting the medical care you need and deserve.

I have suffered greatly for the past 5 years, due to my extensive turbinectomy surgery, that was performed when a surgeon removed all of my turbinates. I've had the privilege to work with Chris Martin, and David Lemberg, just to name a few. I have done my best to represent every ENS sufferer that struggles with this condition, while working with the doctors at Mayo Clinic. I believe what has transpired in the past year, is just the beginning of great things to come, but we need to take action in this study, in order for it to advance scientifically!

Another doctor, I have great respect for also is Dr. Amit Sood, he has impacted my life greatly with his coping techniques and meditation, he practices bio-feedback breathing, along with relaxation techniques in order to improve ones quality of life. Dr. Friedman's main objective is to improve symptoms in patients so he can further his research, that Dr. Eugene Kern established years ago.

When you arrive at Mayo Clinic, you will find that the doctors and staff not only show compassion, but exhibit concern for your condition. You can also be assured that you will be respected and treated with quality care. Furthermore, because of the many years of dedication and research by Dr. Kern, ENS is well recognized and accepted at Mayo Clinic. From my own perspective, Mayo Clinic has been the only hospital where doctors have understood my condition, in the past I've had doctors waste my time, money and then was told I have no further options. When you leave the study at Mayo Clinic, you won't just leave empty-handed, but will leave with hope, encouragement, and tools to cope, and hopefully a new perspective on life.

"Think of the possibilities, that one day having victory over Empty Nose Syndrome, could become a reality"

I look forward to your participation,

Sincerely yours,

Paul

Advocate and Fellow ENS Sufferer

Dear Paul

I am grateful for your continued support of our mission - to help the countless patients who suffer from the devastation of Empty Nose Syndrome. I really feel sense of urgency to complete this project because I understand how much suffering our patients have every single day, and each day that passes is another day of suffering for so many. This is why I am so thankful for your help.

As we have discussed on numerous occasions, it saddens me greatly to know that there are patients across this country and around the world who have been treated in a dismissive way by physicians who have been unable to help them. As a physician, I tell you that it is very frustrating to be unable to provide the patient with a rational, scientifically based treatment regimen for processes like Empty Nose Syndrome. I'm confident that our treatment plan will succeed in reducing the suffering.

I am so happy that our common goal of helping people with this devastating condition has brought us together. Thank you very much for your help in our pursuit of a scientifically sound treatment plan for patients with Empty Nose Syndrome. I look forward to seeing you again in the future.

Sincerely,

Oren Friedman, M.D.

Pre-Clinical Studies

UBM as a Laryngeal Repair / Replacement Scaffold

Pilot studies were conducted utilizing UBM as a scaffold for the replacement of completely resected vocal folds in a dog model. Results of this pilot study showed that there was reconstitution of the vocal fold with complete epithelialization over the surface of the fold, in-growth of innervated skeletal muscle into the fold and at least partially organized connective tissues constituting the submucosal portion of the fold. The submucosal portion of the fold is important for vocalization.

A second study utilizing the use of UBM as a laryngeal replacement device was conducted under SBIR grant funding. Dr. Peter Hilsamer performed complete hemilaryngectomies on ten animals with appropriate controls. A multilaminate sheet of UBM material was used to replace the laryngeal wall and a single layer of UBM was used to reconstruct the vocal fold. The study concluded that UBM remodeled into organized cartilage and skeletal muscle over a 2-3 month period and vocalization returned within four to six months. The results of this study are pending publication.
